Upper Cervical Spine - Odontoid Type 2B Posterior Oblique Fracture in the Young.

19 yrs young engineering student unfortunately following a RTA / MVA sustained Axis C2 Type 2B fracture. He was ASIA -C on presentation to ER late evening after an interstate transfer.

Initial stabilisation was achieved following ATLS protocols. Appropriate radiographs and trauma CT Spine was done which showed a displaced C2 Type 2B fracture. Reduction was achieved immediately and MRI cervical spine was done which demonstrated reduced fracture and Cord Myelomalacia changes at C2 (image 3). Extensive family counselling was done about injury, treatment plan and prognosis. He was taken up for surgery the following day. 

Orthopaedic Spine Surgery : 

- Biplanar C-Arm was used.

- Anterior C2 Stabilisation with 36 mm Canulated Partially threaded 4.5 mm screw. Compression at fracture site was achieved with lag effect. 

Post Op Neck was immobilised in a Hard Cervical Orthosis and Spinal Cord Injury Rehabilitation has been commenced.

Mrs. G.S 44 yr homemaker was seen in my practice 18 months ago. She presented to me in a non ambulatory state with quadriparesis due to C3 instability and epidural abscess caused by Mycobacterium Tuberculosis.

Mrs.R S, had a long standing thoracic spinal cord compression due to a condition called OPLL - Ossified Posterior Longitudinal Ligament. Adult Isthmic HSG - High Grade Spondylolisthesis 
-Spondylolisthesis is Forward slippage of one vertebral body over the vertebral body below it.

- In an adult is caused by a defect i.e stress fracture in the pars interarticularis . - Pars Defect i.e stress fracture is seen in 4-6% of the general population and 85% occur at L5/S1 level and 15% occur at L4/5 level.

- Meyerding Grade 1-2 slips less than 50% are Low Grade Spondylolisthesis - LSG and Grade 3-5 slips more than 50% are High Grade Spondylolisthesis -  HSG.

-Mrs. E.N, 36 yrs young homemaker came to me for treatment as she was suffering for a very long time with instability related low back pain and both leg sciatica i.e Neurogenic Claudication. She was unable to stand/walk even for short duration at home. Her quality of life was very poor. - She had High Grade Spondylolisthesis i.e Stress fracture of Pars and forward slip at L5/S1 level .

Lumbar Chronic Disc Prolapse Spine Surgery Mrs.S had a large lumbar disc prolapse for which she has seen and got non operative treatment elsewhere. Her posture and gait gradually got worsened over time and it had an impact on her social life as well. She presented to me with worsening symptoms of pain, gait changes & gross postural deformity. She had a high BMI and after detailed clinical examination, Lumbar Spine Surgery was performed . She has responded well to Orthopaedic Spine Surgery . She understands that she needs to reduce her BMI in the future during her rehabilitation phase. High BMI should not be a contradiction for Orthopaedic Spine Surgery as in carefully selected patients it gives good clinical results. 15 yrs old teenager following a MVA sustained a subaxial cervical spine injury. C7 Unstable Flexion Teardrop Fracture with PLC injury. His SLIC severity score was more than 5. He had Right C8 sensory deficit with brisk reflexes. Motor was intact C5-T1 & L2-S1. 

Orthopaedic Spine Surgery : Anterior Cervical C7 Corpectomy, Decompression and Fusion from C6-T1 with expandable cage and anterior cervical contoured plate. 

Patient and family were explained the possible need for posterior stabilisation and fusion in future. Since his facets were not fractured only anterior decompression and rigid stabilisation was performed. 

He recovered well and was mobilised the very next day with a hard cervical collar.

Mr.ABR , 47 yrs young welder had been suffering from upper back pain due to calcified disc prolapse for a long time causing severe nerve pain in both his legs while walking.
His scans showed a calcified disc at L1/2 causing Severe Lumbar Canal Stenosis . He underwent Orthopaedic Spine Surgery L1/2 TLIF ( Decompression and Fusion Spine Surgery ).
He was able to walk pain free the very next day and was quite pleased with the outcome of his Spine Surgery.
** Upper lumbar disc herniations especially L1/2 level have been reported to occur with a frequency of less than 1% of all disc herniations. Early diagnosis and appropriate procedure in selected patients give good clinical results.

Cervical Spine Surgery in an 86 Yrs Old Senior Citizen

Mr. S.V presented with symptoms of Severe Cervical Spondylotic Myelopathy ( weakness of upper and lower limbs with gait changes) caused by Multilevel OPLL ( Ossified Posterior Longitudinal Ligament ) which causes weakness of upper and lower limbs with gait changes.
He underwent Posterior Cervical Spine Surgery ie Posterior Cervical Decompression Stabilisation and Fusion from C3 - C6 .
He was mobilised on the first Post Operative day and is making good clinical Progress .
   Age should not be a deterrent factor in an appropriately selected patient for Orthopaedic Spine Surgery. Good Clinical examination and appropriate investigations along with through patient and family councelling and safe spine surgery with a good team are important factors for great clinical outcomes .

Metastatic Spine Tumor SurgeryT10 Decompression, Anterior Reconstruction with Expandable Cage, Stabilisation and Fusion from T8-T12.

26 yrs old young male who was diagnosed to have mediastinal germ cell tumor and had Tumor removal Surgery few years back presented now with ongoing severe back pain on mobilisation. On further investiagtions he was noted to have Oligometastasis to T10 with complete destruction of T10 Vertebral body. On clinical examination his motor power in his legs ( L2-S1)  were preserved.

46 yrs old lady presented with progressive kyphotic deformity ( Hunch back ) due to her condition of Ankylosing Spondylitis.

June is Scoliosis Awareness Month

Scoliosis is a condition that causes the spine to abnormally curve sideways, into an “S” or “C” shape.

- Although many people have not heard of the condition it is surprisingly common, impacting infants, adolescents and adults of all races, classes and both genders.

- Approximately one out of every six children diagnosed with scoliosis will have a curve that requires active treatment, sometimes involving surgery.

 Corona & Other Infection Control Measures for Health Care Professionals and General Public 

- Wash your hands often with soap and water for at least 20 seconds, especially after going to the bathroom; before eating; and      after blowing your nose, coughing, or sneezing.

- If soap and water are not readily available, use an alcohol-based hand sanitizer with at least 60% alcohol. Always wash hands with soap and water if hands are visibly dirty.

- Regular Hand washing with copious amount of water and soap after each patient contact .

- Cover your cough or sneeze with a tissue, then throw the tissue in the trash bin.

- Clean and disinfect frequently touched objects and surfaces using a regular household cleaning spray or wipe.

- Avoid close contact with people who are sick.

- Avoid touching your eyes, nose, and mouth.

- Seek appropriate medical help if you have symptoms of fever /cough/ myalgia ( muscle aches).

Mr.Laxman Sahu had undergone 2 previous Lumbar Spine Surgeries ( Minimally Invasive and Endoscopic Spine Surgery) .He was in significant incapacitating pain and his quality of life was reduced significantly reduced when he consulted me .

His medical history has severe co-morbidities.He had undergone Nephrectomy ( Single Kidney Patient ) and had Cardiac angioplasty ( heart stents ) .On anaesthesia assesment he was classified as high risk for surgery (ASA 4) .

Adult Isthmic HSG ( High Grade Spondylolisthesis)

- Spondylolisthesis ( Forward slippage of one vertebral body over the vertebral body below it ) in an adult is caused by a defect (stress fracture) in the pars interarticularis (Link) .

- Pars Defect (stress fracture) is seen in 4-6% of the general population and 85% occur at L5/S1 level and 15% occur at L4/5 level.

- Meyerding Grade 1-2 slips(< 50%) are Low Grade Spondylolisthesis (LSG) and Grade 3-5 are High Grade Spondylolisthesis (HSG).

 Mrs.Manjula 51 yrs old homemaker from Andhra Pradesh came to me for treatment as she was suffering for a very long time with instability related low back pain and both leg sciatica (Neurogenic Claudication). She was unable to stand/walk even for short duration at home. Her quality of life was very poor.

- She was unfortunate to have both Spondylolysis ( Stress fracture of Pars ) at L4 level and High Grade Spondylolisthesis ( Stress fracture of Pars and forward slip ) at L5/S1 level .

-Orthopaedic Spine Surgery : Posterior L5/S1 Reduction of slip and TLIF ( Transforaminal Lumbar Interbody Fusion ) and L4/5 Stabilisation and PLF (Postrolateral Fusion).

- Patient was mobilised within 24 hrs of her surgery at Yashoda Hospital Somajiguda.

Mr. Andrew Sakala, retired Airforce pilot from Zambia who had 2 previous unsuccessful spine surgeries overseas presented with infected failed back surgery syndrome. He was incapacitated with pain, loss of function and inability to walk. He underwent 2- stage, 360 degrees back and front revision spine surgery at Yashoda Hospital, Somajiguda. 

First stage involved removal of previous infected implants, thorough debridement, decompression and extended fusion with cemented reimplantation of screws. Second stage involved anterior retroperitoneal lumbar decompression and tricortical iliac crest (autograft) anterior strut fusion to achieve anterior stability. He had 6 weeks of intravenous antibiotics following surgery.

 He made a remarkable recovery and was able to walk independently and is pain free. Very good team effort and result at Yashoda Hospital Somajiguda, Hyderabad.
