Upper Cervical Spine - Odontoid Type 2B Posterior Oblique Fracture in the Young.
19 yrs young engineering student unfortunately following a RTA / MVA sustained Axis C2 Type 2B fracture. He was ASIA -C on presentation to ER late evening after an interstate transfer.
Initial stabilisation was achieved following ATLS protocols. Appropriate radiographs and trauma CT Spine was done which showed a displaced C2 Type 2B fracture. Reduction was achieved immediately and MRI cervical spine was done which demonstrated reduced fracture and Cord Myelomalacia changes at C2 (image 3). Extensive family counselling was done about injury, treatment plan and prognosis. He was taken up for surgery the following day.
Orthopaedic Spine Surgery :
- Biplanar C-Arm was used.
- Anterior C2 Stabilisation with 36 mm Canulated Partially threaded 4.5 mm screw. Compression at fracture site was achieved with lag effect.
Post Op Neck was immobilised in a Hard Cervical Orthosis and Spinal Cord Injury Rehabilitation has been commenced.
