Adult Isthmic HSG ( High Grade Spondylolisthesis)

- Spondylolisthesis ( Forward slippage of one vertebral body over the vertebral body below it ) in an adult is caused by a defect (stress fracture) in the pars interarticularis (Link) .

- Pars Defect (stress fracture) is seen in 4-6% of the general population and 85% occur at L5/S1 level and 15% occur at L4/5 level.

- Meyerding Grade 1-2 slips(< 50%) are Low Grade Spondylolisthesis (LSG) and Grade 3-5 are High Grade Spondylolisthesis (HSG).

 Mrs.Manjula 51 yrs old homemaker from Andhra Pradesh came to me for treatment as she was suffering for a very long time with instability related low back pain and both leg sciatica (Neurogenic Claudication). She was unable to stand/walk even for short duration at home. Her quality of life was very poor.

- She was unfortunate to have both Spondylolysis ( Stress fracture of Pars ) at L4 level and High Grade Spondylolisthesis ( Stress fracture of Pars and forward slip ) at L5/S1 level .

-Orthopaedic Spine Surgery : Posterior L5/S1 Reduction of slip and TLIF ( Transforaminal Lumbar Interbody Fusion ) and L4/5 Stabilisation and PLF (Postrolateral Fusion).

- Patient was mobilised within 24 hrs of her surgery at Yashoda Hospital Somajiguda.
